Proof of Age for the Minor

  • Birth certificate is the most commonly accepted document.
  • School ID card with date of birth is also acceptable.
  • Passport of the minor can serve both as ID and age proof.
  • The document must clearly show the minor’s name and date of birth.
  • Original and a self-attested photocopy are typically required.

Photographs of the Minor

  • Recent passport-size color photograph of the minor is needed.
  • Usually 1–2 photographs are submitted depending on bank policy.
  • Photograph should be recent and match official documents.
  • In some cases, both minor and guardian’s photos are attached to the form.
  • Digital images may be taken at the bank branch if required.

KYC Documents of the Guardian

  • Aadhaar card and PAN card of the guardian are mandatory.
  • Address proof such as passport, utility bill, or voter ID must be submitted.
  • These documents establish the guardian’s identity and residence.

  • The guardian must be either a parent or a legal guardian.

Relationship Proof Between Minor and Guardian

  • The bank may request a declaration form confirming the relationship.
  • Verification is essential for establishing lawful guardianship.

Account Opening Form and Declarations

  • Duly filled minor account opening form signed by the guardian.
  • Selection of account type (savings, FD, RD) and mode of operation.
  • Nomination form for future succession (optional but recommended).
  • Declaration that the minor is not operating another account, if required.
  • Banks may also ask for an undertaking for fund usage on behalf of the minor.
